Cosmetic fillers are a non-surgical, minimally invasive, medical treatment that uses a gel-like injected into the skin to correct creases, wrinkles, and folds associated with aging. They also add volume and definition by altering contours to regain a more youthful appearance.

Injectable hyaluronic acid (HA) is a type of temporary dermal filler. Hyaluronic acid is a naturally occurring sugar found throughout the body, with the highest concentrations in the joints, eyes, and skin. In the skin hyaluronic acid attracts water, which allows the skin to become more hydrated, but, as we age, the fat, muscles, bone, and skin in our face begins to thin. This loss of volume leads to either a sunken or sagging appearance of the face, fine lines, wrinkles, folds, and thin lips. Injectable HA is used to keep our skin hydrated and to create volume through its ability to bind water molecules— reducing the appearance of fine lines and wrinkles, facial folds, and creating structure, framework, and volume to the face and lips.

The effects of injectable hyaluronic acid are seen immediately, and also improve months after injection due to the increase in hyaluronic acid production in the treatment area. Injectable HA fillers are not permanent and retreatment is needed to maintain your results, they can last around 9-24 months (depending on the area treated, composition, and patient characteristics). Injectable HA can help volumize cheeks, smooth smile lines, smooth lines around the mouth, plump the lips, define the chin, improve the jawline, or improve the appearance of undereye hollows.

Cosmetic fillers can be used for the beautification of the face by improving facial proportions, this is done to create a more defined and contoured look by adding small amounts of filler in the correct places.

BioStimulators — RADIESSE® | Sculptra®

Biostimulators are dermal fillers that promote the body’s natural production of collagen and elastin over a period of several months. The fillers themselves act as tiny scaffolds for these substances, which add volume to treatment areas and help reduce the appearance of lines and wrinkles with improvement of the skin in the area overlying the injection site.

Unlike HA fillers which are used to rehydrate and create volume, biostimulators work by stimulating new tissue growth, boosting your body’s natural production of collagen for natural-looking and prolonged results as the newly produced collagen is permanent. Stimulation of collagen can help tighten or at the very least maintain your skin’s turgor & tensile strength — this prevents skin laxity & skin sagging. The visible results of biostimulatory injectables last 12 to 24 months, therefore requiring less maintenance than HA fillers.

Both calcium hydroxylapatite (CaHa) and poly-L-lactic acid (PLLA) fillers are considered biostimulators. The most popular CaHa and PLLA brands are Radiesse® and Sculptra®:

  • Calcium hydroxylapatite, or CaHA, is what makes Radiesse unique as an injectable aesthetic treatment. It’s the first and only CaHA injectable available that provides both immediate, natural-looking results and long-term improvement—a year or more in many patients. Radiesse provides immediate contouring plus stimulation of collagen and elastin production that contribute to the skin’s strength and elasticity. Unfortunately, aging and environmental factors (think sun damage) make it harder for your body to produce these components. Reduced amounts of collagen and elastin can cause facial skin to wrinkle and sag, stealing from your youthful appearance.

    Upon injection, the Radiesse CaHA gel matrix provides immediate improvement, CaHA particles then integrate into your skin and create a “scaffold” that stimulates the production of collagen and elastin.   • Sculptra addresses an underlying cause of facial aging, not just the symptoms. Sculptra is made with a biocompatible, biodegradable, synthetic material called poly-L-lactic acid, which is gradually and naturally absorbed by the body and helps to rebuild lost collagen through a series of treatments. Sculptra works with your body within the deep dermis to help revitalize collagen production and restore your skin’s inner structure and volume to diminish facial wrinkles.

    Over time, Sculptra helps the body reproduce lost collagen. The number of injection appointments and the number of injections per appointment vary from person to person, depending on how much collagen has been lost. On average, three injection appointments are needed over the course of 3-4 months, to maintain results we recommend that you repeat the procedure every 2 years. PRP (Platelet Rich Plasma)

Platelet-rich plasma (PRP) therapy is a form of regenerative medicine that harnesses and amplifies the natural growth factors found in our blood cells to help heal damaged tissue. Unlimited clinical applications and long history as a safe, low-risk autologous treatment have contributed to its increasing popularity for people looking for a more natural treatment.

Platelet-rich plasma (also known as PRP) is blood plasma containing a high concentration of platelet. Platelets (also called thrombocytes) contain clotting and numerous growth factors, vital for tissue regeneration and repair which helps to stimulate new collagen. The growth-promoting properties can be leveraged to accelerate the repair of aged and damaged tissues, with common applications including facial rejuvenation and hair loss. Only pure and highly concentrated PRP can release the quantity of growth factors required to stimulate cellular regeneration. At Esteem, we use World PRP, an innovative and unique PRP collection system that extracts pure, concentrated, and unique platelets in high quantities that can be customized to esthetics and medical treatments.

PRP treatments are done by spinning a small volume of the patient’s own blood through a centrifuge. This separates and concentrates the blood’s components, including platelet-rich plasma and the “buffy coat,” a solution that contains immune cells. The provider combines these two components with a small amount of calcium chloride (which activates and keeps the PRP stable), then injects them into the treatment area. Over a period of months, PRP stimulates the body’s natural collagen production.

Injectable PRP can be done in multiple areas of the face and body to stimulate tissue regeneration:

 
  • PRP injections in the under-eye area (administered by our medical professionals) can treat fine lines and wrinkles and dark circles by stimulating new collagen production for optimal rejuvenation. The skin in the undereye area tends to be sensitive and thinner with less fat underneath it, meaning it is more delicate than the skin on the other parts of your face. These factors mean that the eye area is often the first to show signs of aging and fatigue. PRP under-eye treatment is an excellent way to treat dark circles, tighten loose skin, and increase blood flow to this region. Treatment is typically done in 3 cycles once a month and results generally last up to 1-2 years, maintenance is recommended every 6 months.

    Fotona SmoothEye with PRP — A non-ablative laser treatment that is used to tighten loose and aging skin of the eyelids and periocular region. The intense, controlled surface tissue heating stimulates collagen remodeling and initiates neocollagenesis. Many clients see an instant improvement due to stimulated collagen contraction, but these will continue to develop, with optimal results visible after 3-4 months.     What about HA Fillers?

    The undereye area is delicate which is why we promote PRP undereye injections over the use of HA fillers. Some people are not candidates for HA filler, for example, someone who is older with larger bags, lots of excess skin, pigmentation, and/or very thin skin, injecting HA filler can actually worsen these attributes. HA fillers can sometimes refract light beneath the skin, giving the under-eye area a blue tint (called the Tyndall effect). Rather than volumizing the under-eye area with HA filler, PRP simply encourages your skin to rejuvenate itself by generating collagen, elastin, and natural hyaluronic acid. While it does take longer to see results, PRP is a safer and more effective long-term treatment than HA fillers in most patient circumstances.

  • A scar is composed of fibrous tissue and may be formed for many different reasons, including as a result of infections, surgery, injuries, or inflammation of tissue. PRP therapy is an innovative treatment option for scarring that can help enhance and improve the appearance of skin by healing the scars and improving circulation. Scars can be treated with PRP injections or in combination with other treatment therapies like microneedling, RF microneedling, and fractional resurfacing.

    Acne Scars (atrophic scars) — Pitted or indented scars that occur when your skin heals below the normal layer of skin due to a lack of regenerative tissue. PRP has efficacy in the management of atrophic acne scars. It can be combined with microneedling to enhance the final clinical outcomes in comparison with microneedling alone.

    Keloid Scars — Generally caused by skin tissue growing over inflammation and are considered permanent making it the most challenging type of scar to treat. PRP has been proven to aid and repair scar formation and has been used to advance keloid treatment.

    Hypertrophic Scars — PRP has been shown to help with wound healing and the appearance of hypertrophic scars. When combined with a specific type of laser, the scar’s color, shape, and texture improve dramatically.

  • Many global studies prove that PRP therapy is considered one of the most successful non-surgical hair loss treatment methods. PRP treatment is considered safe and highly effective in preventing hair loss and hair thinning and also works to regrow hair and promote thicker, healthier hair growth. There are various types of hair loss conditions that can occur, which can cause the hair follicles to become weak and inactive. Silhouette InstaLift® threads are a non-surgical, in-office procedure that uses dissolving thread sutures applied deep to the skin to reduce the appearance of aging skin to the mid-face or jawline area. It is only non-surgical rejuvenating procedure that uses advanced Micro-Suspension Technology™ to offer immediate and lasting results— up to 2 years. Threads are made from poly lactic-co-glycolic acid (PLGA), a well-known biomedical copolymer that is biocompatible and biodegradable, that feature hand-tied knots and bi-directional cones. BELKYRA® — Liquid Lipo

Belkyra® (also known as Kybella®) is a revolutionary new treatment that destroys fat cells. The active ingredient in Belkyra is called deoxycholic acid, which is a naturally occurring molecule in the body that assists with metabolism and the breakdown of fat. Belkyra works by disrupting the cellular membrane of fat cells, causing apoptosis or cell death to occur. The dead cells and fat are then cleaned out of the area by your body’s own waste-removal system over time.

As a treatment, Belkyra is injected into the submental fat beneath the chin (double chin) destroying fat cells, resulting in a noticeable reduction in fullness under the chin. Once destroyed, these cells can no longer store or accumulate fat, so further treatment is not expected once you reach your desired aesthetic goal. Belkyra is only approved for localized fat removal and may require a number of treatments to achieve the desired results.

The entire procedure takes about 15 to 20 minutes and is very comfortable for the patient. Results are seen around 2-3 months and most patients will require 2-4 treatments (spaced 4-6 weeks apart) to achieve their desired results— once achieved, re-treatment is not expected. Since everyone's chin profile is different, the number of treatments varies from patient to patient.
